Key information
You will learn the theory and practice of designing, planning, implementing and evaluating multi-disciplinary services to improve the health of individuals and communities. You will gain a thorough grounding in the key concepts and models of integration and personalisation as applied to health and social care, and the values, principles, evidence and theories underpinning them.

The programme is structured around the key components of successful integrated care development and implementation:
establishing purpose and need engaging individuals and communities leading self and others managing change reviewing and evaluating working with service users and carers working with staff working with processes (eg information technology) sustaining and improving.
You will select your elective modules from the wide range of Continuing Professional Development options offered by the School of Health Sciences, covering areas including:
health policy public health health and social care commissioning.

為來自中國的學(xué)生設(shè)計(jì) Applicants will be considered for most postgraduate courses with a good Chinese bachelor’s degree from a recognised University. Students who don’t meet the requirements for direct entry may opt to undertake a graduate preparation programme. Students successfully completing INTO City’s Graduate Diploma or Kaplan International College London’s Pre-Master’s Course at the required level will have guaranteed entry into City’s postgraduate degrees. English language requirements will vary from course to course and the information on this page is only intended to be a general guide.
